The All-Wales COVID-19 data collection tool is a one-page tool that will enable Wales to amass all of the data from all of the hospitals at the end of this pandemic.
The form should be printed out and filled in for every patient who is admitted to hospital with COVID-19. Each hospital should have a person who is designated to collect this data, and every hospital should have somewhere to store the completed forms.
